I haven't played the newest version, which seems to be called Ultimative, but I've played various XL versions throughout the years. The uberquests are incredibly awesome and suitably hard. I never even did some of the impossible level ones. You can also improve the graphics hugely by using a widescreen mod, just don't abuse it so you can hit creatures for what would be off-screen. Yes, the original creator for Median was Brother Laz, who also made a lot of item packs for PoE. Laz went on to make Median 2008, and then XL, and then he quit and passed it on to MarcoNecroX who has been making the current Median versions including Ultimative XVc/XVr(Realm only).
He and another developer 'whist' have been working on a new version of Median called 'Sigma' which will completely rewrite the code of Median to make the engine a whole lot better. It looks very promising. As for widescreen, there's already D2Multires which is fairly easy to implement and use. PlugY offers infinite shared and personal stash space which works very well with both MultiRes(which is btw banned in multiplayer realm called The Sin War), and with the mod.
I never really got as far as you did, I think the hardest I finished was maybe Primus or Toraja with my Trap Rat Druid and Retaliator Paladin. They have some amazing concepts that could very well be implemented in Grim Dawn, in time perhaps. Their end game model is something really amazing.
Whole new sublevels filled with newly designed mobs of varying strengths and weaknesses, bosses that have heroic shields that only drop a certain immunity every now and then, bosses that are completely immune to all damage except a skill called 'Purify' which is obtainable through one of the earlier uberquests, bosses that have to be killed in a very specific way, etc. Stuff like this is real end game. It's a pity that the game isn't played by a lot of people though, probably because of Diablo 2's bad graphics. Still the devs have been incorporating a lot of new models, spell animations and the works which look pretty damn good. If you've used d2se, then downloading and installing the mod should be no problem as it uses that program. In addition you can use plugy for the mod as well. It is hands down my favorite arpg of all time. Fundamentally it is a completely new game compared to diablo 2. All new skill trees, 100's of hand crafted unique and sets, Uber Levels, Unique Charms, and character customization. Also the end game, like the other poster stated, is absolutely amazing there are a variety of different 'uber dungeons' each with a specific theme. It's definitely worth a download IMO.

Diablo 2 Median Xl Hack
For anyone who's interested in checking this out, here's the website. Besides a working copy of Diablo 2, all you need to install and start enjoying the mod is there, along with a community forum, and most importantly the game's documentation which explains everything about how it works. There's also a launcher for the mod that keeps it updated. I'm not sure if it's officially released, but I've been using it with no issues. The video does a good job at giving a general overview of what's new in the mod. For a more personal example, I'm just getting to act 5 with a Shuriken Flurry assassin which has been fun. I've been farming Destruction difficulty (Hell difficulty) with a level 120 necro that uses minions and fire/cold damage dealing totems.
I've probably completed about half of the endgame content with that one character. There are always public games open, usually a few people hanging out in chat or discord. Sure cuts a lot serial number mac. If anyone here actually checks it out and needs a friend, feel free to send me a pm here or on the forum. I'm Shovelspoon on the forum and in-game as well.
